Use this form to report abuse, neglect, and/or exploitation of
vulnerable adults.
A vulnerable adult is someone 18 years and older who because of a mental, developmental, or physical impairment, is unable to:
- 1. Communicate or make responsible decisions to manage one's own care or resources.
- 2. Carry out or arrange for essential activities of daily living.
- 3. Protect oneself from abuse.
